How To Schedule Your Add-on Medical Care
- Select your state and medicare enrollment status below.
- Choose a doctor you would like to work with. Not sure who to pick? Read about our qualified doctors in Learn more about me.
- After selecting your doctor, click on the Schedule An Appointment button underneath their photo.
- Scroll down the “Select a Service” list and pick the option “Plant Docs Jumpstart Bundle”
- From the calendar, select your appointment date and time. Make sure you check the time zone listed as it may not be the same time zone you live in.
- Complete the patient registration form, and click the confirm button.
You did it! Your first appointment is now scheduled. You will receive instructional emails explaining how to set up your patient portal, answer your new-patient questionnaires, and prepare for and join your appointment.
Please Note: You must select a doctor that is licensed in the same state you will be in during your scheduled appointment. Unless you will be traveling, we recommend choosing a physician licensed in your home state.
